A week before a traffic accident Donald Sr. got sprayed in the face with Ammonia. Some one removed the O-ring from the valve assembly. This is the position Don's face was in when he turned the valve, note the field wiped clean. He made it to the ditch to wash his eyes and had to stop and wash again in a puddle before receiving help. 

Don said the valve was super-tight, causing him to lean into it, then it sounded like a shot.
